Liability Waiver
I understand that participation in fitness programs and training sessions at New Moon Fitness involves physical activity and carries a risk of injury.
I certify that my child is physically able to participate in the Rising Strong program and I assume full responsibility for any risks, injuries, or damages that may occur as a result of their participation.
I understand that all instruction is designed to be safe and appropriate for each participant, but I acknowledge that injuries can occur in any physical activity.
I release and discharge New Moon Fitness, its owner, and instructors from any liability, claims, or causes of action arising out of my child’s participation in this program.
